Position: NURSE PRACTITIONER, M.S. /40HR DAYS - HEMATOLOGY / MEDICINE

Summary: Provides clinical care, monitoring and follow-up for Hematology patients consistent with BWH nursing policies, procedures and standards. Acts as a liaison between various areas of patient care to ensure continuity. Provides support to Hematology physicians in all aspects of patient care. Participates in all aspects of clinical research protocols.

Principal Duties and Responsibilities:

Works in the Hematology outpatient clinic at DFCI providing Nurse Practitioner support and develops, implements and evaluates a program of specialized health care for select patient populations. Promotes a collaborative relationship and effort between the medical staff, professional nursing staff and clinic staff toward continuity of patient care and efficient use of resources. Serves as a role model for professional nursing practice.

Provides direct care to select patient populations (i.e., hemoglobinopathies, bleeding disorders, clotting disorders, anemias of various etiologies) in collaboration with the physicians of the BWH Hematology Division. Performs and records the patient history and physical exam and discusses findings with the physicians. Utilizes advanced nursing knowledge to identify, prevent or solve complex and/or recurring patient care problems.

Follows and manages patients (in collaboration with MDs) enhancing continuity of care in both the outpatient and inpatient settings.

Performs medically delegated aspects of patient care in accordance with practice guidelines. Collaborates with clinic staff and physicians to facilitate a comprehensive plan of care and coordinate resources.

Writes orders that are in accord with practice guidelines. Maintains and practices in accord with practitioner guidelines per state and institution(s) regulations. Provides consultation to both nurses and physicians.

Serves as a resource to staff in area(s) of expertise. Serves on appropriate departmental and interdepartmental committees. Participates in the development of formal and informal educational programs for nursing staff.

Collaborates with Clinical staff Educators, Nursing Educators and other health care professionals to meet staff and patient education needs.

Monitors patient outcomes in accordance with defined standards of patient care. Collaborates with nurse managers, clinical nurses and medical staff to plan, implement and evaluate pertinent changes in practice and health care delivery.

Facilitates patient movement through the system and efficient use of resources through interdepartmental collaboration. Supports and participates in clinical research efforts to advance knowledge and to promote research-based practice. Evaluates own practice and program effectiveness.

Works as a member of the inpatient Hematology team. Serves as a resource for nursing and medical staff to facilitate continuity of care for patients and to meet staff and patient education needs. Collaborates with Care Coordination staff for case management and discharge planning.

Qualifications

Master's degree in Nursing required.

Satisfactory completion of an approved formal education program which has as its objective the preparation of professional nurses to practice as nurse practitioners. Ambulatory care preferred.

ANA or other appropriate nursing certification in area of practice.

Authorization by the Massachusetts Board of Registration in Nursing to practice in the expanded role.

DEA for prescription writing.

Experience in providing care to Hematology patients in both in-patient and out-patient settings preferred

Experience in pain management and chronic disease preferred

Experience in clinical research study management preferred

Skills and abilities

Thorough knowledge of benign and malignant hematology patient-care issues is highly desirable.

Ability to work collaboratively and function as an effective member of the health care team

Ability to serve as a resource for medical, nursing and support staff regarding hematology patient management and Factor administration
